Description
Percentage of patients aged 12 years and older screened for clinical depression on the date of the visit using an age-appropriate standardized depression screening tool, and, if screening is positive, a follow-up plan is documented on the date of the positive screen. The Depression Screening and Follow-up measure was revised in 2016 and is not comparable to prior years. This measure was aligned with a specific clinical measure, CMS2v11, used by the Centers for Medicare and Medicaid Services.
