Description
Percentage of medical patients aged 18 years and older with a documented BMI during the most recent visit or during the previous twelve months prior to that visit. The patient is counted if they had a BMI outside of normal parameters and a follow-up plan was documented during the visit or during the previous twelve months of the visit. The Body Mass Index (BMI) Screening and Follow-Up Plan measure was revised in 2019 and is not comparable to prior years. This measure was aligned with a specific clinical measure, CMS69v9 used by the Centers for Medicare and Medicaid Services.
