Description
Percent of all patients of a health center that identified a school-based health center as a service delivery site in their scope of project (grant or designation) are to report the total number of patients who received primary health care services at the approved school service delivery site(s). A school-based health center is a health center located on or near school grounds, including pre-school, kindergarten, and primary through secondary schools, that provides on-site comprehensive preventive and primary health services. Services are targeted to the students at the school, but may also be provided to their children, siblings, or parents and may occasionally include persons residing in the immediate vicinity of the school.
