Percentage of patients aged 21 and older with a diagnosis of clinical atherosclerotic cardiovascular disease (ASCVD) or a cholesterol (LDL-C) level greater to or equal to 190 mg/dL, or percentage of patients age 40-75 with a diagnosis of diabetes with an LDL-C level of 70-189 mg/dL. This measure was aligned with a specific clinical measure, CMS347v5, used by the Centers for Medicare and Medicaid Services.
